Continue to use your VRI PERS devices by signing up for self pay.

Your coverage through your health plan has ended

Your eligibility for the Personal Emergency Response System (PERS) through your health plan has ended. You have the option to keep your device through self pay by paying a monthly fee for yourself or a loved one. To sign up, fill out the form below or call the VRI Care Center at 855-295-3131.

Why is my PERS service ending?

Your health plan is no longer offering the PERS device and service. That means your device will be disconnected and you can’t use it to call for help, unless you decide to continue the service for a fee. If you have questions about your coverage, please contact your health plan directly. 

What is the cost of self pay?

The cost to continue to use your devices through self pay is $27 per month for in-home devices and $32 per month for mobile and watch devices. You will keep using the same device you already have.

How will I be billed?

After signing up, you will receive your first invoice in the mail. This invoice will contain your account number and methods to access billing. This includes going online, calling our call center, or mailing a check. Auto-payments can be set up through a checking account or credit card.

What happens if I do not continue with self pay?

Since your health plan does not pay for the PERS service any more and your PERS device will be disconnected, we ask that you send your device back to us.

To return your device free of charge:

  • Call 1-855-295-3131 (TTY: 866-825-2242), Monday through Friday, 9am to 5pm
Eastern Time.
  • Once you call, a prepaid package will be sent to you as well as a return label.
  • When you receive the package:
    • Place the device in the provided package and attach the enclosed return label
    • Place the package in your mailbox for your local postal service to pick up
    • Or if you need to have the package picked up in a location other than your mailbox, please visit www.USPS.com and click ‘Quick Tools’. You will be able to schedule a pick up for the time and location that meets your needs
Will the service be the same as I previously had?

Yes, if you continue with self pay you will receive the same service as your current program.

If I don't sign up now, can I sign up later?

Yes, you do not have to sign up now and can sign up when you are ready. However, in the meantime we ask that you follow the instructions to return your device to us.
